Self Service | User Input Type

Could we also include a check box option and radio button option for User Input Type of Self Service?

I would assume that both would use a mechanism similar to the List that we have today where you can have a Caption and then the actual submitted Value. For the check boxes we could do a value if checked and value if not checked.
